Michigan City Coolspring Health Center 1225 E Coolspring Ave Michigan City, Indiana 46360 The Home Health Occupational Therapist (OT) through Franciscan is responsible for evaluating and implementing a plan of care to improve, restoring and enhancing physical functional performance, and promoting injury prevention. This role, also, performs documentation and quality improvement activities, as outlined in departmental policies and procedures. Document patient assessments and pertinent information in a timely manner. Develop a treatment plan with measurable goals based on patient assessment. Provide occupational therapy, including selection and use of adaptive equipment. Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.
