What are the responsibilities and job description for the House Lead position at C&R, Inc.?
Qualifications: One year experience working with adults or children with intellectual disabilities; good organizational skills; have the ability to take direction and delegate tasks to others; good communication skills; high school diploma or GED equivalent.
Responsible to: The House Lead answers directly to their assigned supervisor, the QIDP or the Administrator (or designee).
Work Hours: Hours vary depending on participant needs, but primarily 8a-4p, Monday-Friday; may involve evening and weekend hours as needed, and includes services delivered in the community as well as in the participant’s home and in the administrative office, when necessary.
Essential Functions: Be able to lift 50 lbs.
Maintain CPR/1st Aid certification without lapses.
Maintain current Assistance with Medications (AWM) Certification.
Maintain at least 12 hours yearly training as required by Dept. of Health and Welfare.
Maintain a valid driver’s license.
Maintain auto insurance on private vehicle at all times.
Position requires completion of a criminal history and background check and a subsequent approval/clearance from the Idaho Department of Health & Welfare.
Specific Duties:
· Complete company orientation and ongoing training program.
· Implement program intervention strategies plans as written by QIDP.
· Take and record accurate data and monitor direct care staff’s data regularly.
· Provide individual (1:1) therapy, as needed.
· Meet regularly with the QIDP to receive training on the program and procedures to be followed and to review therapy provided.
· Supervise, in part, all other staff/shifts in participant’s home.
· Help train new staff to all policies & procedures of the specific house/apt.
· Inform supervisor of ANY schedule changes made, getting prior approval for any overtime hours. Assist with staff replacements when they are unable to work.
· Schedule MD appointments and ensure participants get to appointments, including taking Medical Appt. Sheets and turn into the QIDP right away.
· Work with participant’s Payee to ensure participant’s bills are paid on time and other monetary needs are met. Help participants complete and turn in check requests weekly.
· Check and make sure all participant money is accounted for. Take extra funds to the office for safekeeping. Turn all receipts into QIDP with participant’s initials.
· Work with participants by creating and implementing any house rules, cleaning schedules, etc. and train all staff on these rules.
· Provide or schedule participant transportation as needed.
· Prepare equipment and materials needed for implementation of Res Hab intervention strategies.
· Complete written documentation as directed; logs, charts, staff communication.
· Pick up and turn in new monthly and weekly paperwork.
· Ensure correct paperwork is in all books at all times and that tabs, dividers and binders are in good repair.
· Check DSL’s, data sheets and logs/charts daily and make sure they are correct and signed before turning in to the billing & payroll dept. Bring it to staff’s attention when not done correctly.
· Ensure safety of participants in all aspects, ensuring direct care staff also do the same at all times.
· Adhere to all company standards, policies and procedures, ensuring direct care staff also does. Maintain participant confidentiality at all times. Report to supervisor when this is breached.
· Assist/notify supervisor with creating and presenting staff training notes when necessary.
· Update MAR’s (Medication Tracking Sheets) temporarily at direction of Pro-Staff or nurse (if applicable), only until the office can make and send new one.

· Call in refills no less than three days before running out and ensure medications are then picked up from pharmacy.
· Make sure all staff follow doctor’s orders appropriately.
· Communicate with administration about CIR (Critical Incident Reports) circumstances immediately.
· Gather and review Incident Reports for completeness and turn in to office asap.
· Other duties as assigned.
Special Conditions: Position requires that employee/contractor possess a safe driving record and a valid driver’s license. Must be able to lift participants as needed, physically assist in activities of daily living, pull and push a wheelchair, get in and out of the homes of participants. Must be able to make observations of non-verbal behavior and check tasks and activities for completeness. Perform all activities of daily living that are taught. This process must be initiated prior to providing services.
