What are the responsibilities and job description for the RN Consultant (Head Start) position at C.E.F.S. Head Start 0-5 Program?
I. The RN Consultant will provide the following services for the C.E.F.S. Head Start 0-5 Program.
- Develop individualized care plans for children with chronic illnesses as needed.
- Make appropriate referrals to community health resources, agencies, and services to families needing health follow-up care.
- Conduct a home visit to each newborn within two weeks after the infant’s birth to ensure the well-being of both the mother and child.
- Provide needed health education and training to parents and staff.
- Confer with staff and parents about health-related issues as needed.
- Provide expertise, guidance, and support to C.E.F.S. Head Start 0-5 Program’s managers/specialists in specialty area(s).
- Participate in the coordination and/or completion of the program’s annual self assessment and federal review.
- Establish and maintain relationships and collaboration with the medical and dental communities and other community agencies and partners.
- Serve on the Health Services Advisory Committee (HSAC) to guide in the planning, operation, and evaluation of health services.
II. The C.E.F.S. Head Start 0-5 Program will:
1. Reimburse the RN Consultant in the amount of $35.00 per hour for the number of hours requested by the C.E.F.S. Head Start 0-5 Program. Payment will be reimbursed within 30 days after a bill for services rendered has been received.
2. Mileage will be reimbursed at $0.58 per mile as authorized.
3. Provide full access to files and records of C.E.F.S. Head Start 0-5 Program’s enrollees.
III. This independent contract may be terminated, without cause, by sixty (60) days’ prior written notice by either party.
IV. As an Independent Contractor, the RN Consultant will be responsible for his/her own state and federal income, Social Security, and unemployment taxes. C.E.F.S. Head Start 0-5 Program will file the required IRS form 1099.
The Independent Contractor shall provide C.E.F.S. Head Start 0-5 Program proof
professional license, general liability and professional liability insurance coverage through the issuance of a certificate of insurance, listing the Agency as a certificate holder and/or as an additional insured, prior to contractual services being initiated.
