Our organization is currently seeking a Weekend Hospice RN to join our team and provide hospice and palliative care services to our patients. The successful candidate will be responsible for assessing patient needs, developing and implementing care plans, coordinating with other healthcare professionals, and providing emotional support to patients and their families. This position requires a compassionate individual who is comfortable working with patients of all ages and backgrounds. This position is mostly on-call with some scheduled visits.

Responsibilities:

• Provide patient-centered care to hospice and palliative medicine patients.
• Provide emotional and spiritual support to patients and families.
• Assess patient's needs and develop a plan of care.
• Coordinate services with other healthcare providers.
• Educate patients and families about hospice and palliative medicine services.
• Monitor patient's progress and adjust the plan of care as needed.
• Document all patient care activities in the medical record.
• Maintain accurate patient clinical records and reports
  
  
  

Job Type: Part-time

Pay: $1,200 per weekend
