Join By Your Side Hospice Care LLC as a Hospice RN Case Manager, where you will provide compassionate care to patients and their families during life's most challenging moments. In this role, you will be an essential part of a dedicated team committed to enhancing the quality of life for our patients in South Eastern, Wisconsin.
Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans.
- Coordinate and manage patient care, ensuring adherence to hospice policies and procedures.
- Provide direct nursing care, including symptom management and pain control.
- Educate patients and families on end-of-life care and available resources.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Document patient progress and maintain accurate medical records.
- Participate in on-call rotations to provide after-hours support.
- Advocate for patients' needs and preferences in care decisions.
- Current RN license in the state of Wisconsin.
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
- Strong assessment, communication, and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Compassionate demeanor with a focus on patient-centered care.
- Experience with electronic medical records (EMR) systems.
- Certification in hospice and palliative nursing (CHPN) is a plus.
- Willingness to travel within the service area as needed.

Salary : $80,000 - $90,000
