Position Summary
The Clinical Supervisor (RN) is responsible for overseeing the clinical aspects of patient care within the agency. The RN will conduct patient evaluations, review and update care plans, supervise caregivers, and ensure services are delivered safely and in accordance with state regulations.
Key Responsibilities
- Conduct initial patient evaluation visits
- Reevaluate patient needs and monitor patient progress
- Initiate and revise plans of care
- Coordinate patient services with caregivers and other providers
- Supervise LPNs, Home Health Aides (HHAs), and Personal Service Workers (PSWs)
- Conduct supervisory visits in patient homes
- Prepare and review clinical documentation and notes
- Communicate patient status and changes with physicians and healthcare providers
- Provide clinical oversight to ensure safe and effective care delivery
- Review caregiver service plans and documentation
- Ensure agency compliance with Indiana home health regulations
Qualifications
- Active Registered Nurse (RN) license in Indiana
- Experience in home health, community health, or long-term care preferred
- Strong clinical assessment and documentation skills
- Ability to supervise and train caregivers
- Excellent communication and care coordination skills
- Ability to travel to patient homes within service areas
