Job Summary

As a Hospice LPN, you’ll provide hands-on clinical care and support to patients in their homes, while working closely with a multidisciplinary team. You’ll help manage care plans, monitor, and ensure each patient receives the compassionate and skilled care they deserve.

Duties & Responsibilities

  • Providing direct nursing care, including treatments, wound care, and medication administration
  • Monitoring patient conditions and reporting changes to the RN or care team
  • Supporting the development and adjustment of individualized care plans
  • Collaborating with nurses, therapists, and other healthcare professionals
  • Educating patients and caregivers to promote independence and self-care
  • Accurately documenting all care provided and patient updates
  • Participating in team meetings and interdisciplinary coordination

Requirements

  • Equivalent education required for RI licensure required, Bachelor’s Degree (min) in relevant discipline strongly preferred
  • A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or hospice environment setting preferred.
  • Valid CPR certification required.
  • Valid RI Licensed Practice Nurse license
  • Valid driver’s license and automobile liability insurance are required.
  • Ability to read and interpret documents such as medical records, medical reports, etc. Ability to write reports, business correspondence and procedure manuals. Must be able to speak, read, and write English with excellent oral and written communication skills required.
  • Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram or schedule form. Must be able to deal with several abstract and concrete variables.
