Overview
We are currently seeking qualified and reliable professionals to provide Positive Behavioral Interventions & Support (PBIS) and Community Integration Counseling (CIC) services under the New York State Traumatic Brain Injury (TBI) and Nursing Home Transition and Diversion (NHTD) Waiver Programs. The roles involves direct, community-based work with individuals to support independence, stability, and overall quality of life.
Duties
PBIS (Positive Behavioral Interventions and Supports)
- Conduct behavioral assessments and develop support plans
- Monitor progress and adjust interventions as needed
- Provide guidance to staff and informal supports
- Maintain accurate and timely documentation
Qualification
- Bachelor’s degree in a related field required
- Experience with behavioral planning and implementation
CIC (Community Integration Counseling)
- Provide counseling focused on adjustment, coping, and community integration
- Address barriers related to mental health and social engagement
- Maintain clinical documentation in accordance with program standards
Qualifications
- Licensed or license-eligible (LMSW, LCSW, LMHC, or equivalent)
Experience
- Experience working with individuals with disabilities or complex needs preferred
- Strong organizational and documentation skills
- Ability to work independently in community-based settings
- Reliable transportation required
- Familiarity with NYS Waiver Programs is a plus

Pay: $40.00 - $50.00 per hour
