What are the responsibilities and job description for the Help Me Grow Navigator position at Burlington United Methodist Family Services, Inc.?
I. DEFINITION:
The Help Me Grow Navigator will perform administrative duties, technical assistance and facilitate regional home visiting coordinated intake processes, utilizing Help Me Grow, findhelp and home visiting community partnerships. The HMGN will work under the administrative direction of the WVHVP and contracted agency. The HMGN will work closely with both local and State partners on resource/referral linkages, training, support and peer resource sharing, along with regional findhelp coordination.
II. JOB DESCRIPTION:
- Assist with coordination and implementation of all components of the statewide West Virginia Home Visitation Program coordinated intake system.
- Assist with facilitation and coordination of regional home visiting collaboratives.
- Work with both Home Visitation and early childhood partners on outreach to families, hospitals, Drug Free Moms and Babies, child protective services and medical providers.
- Attend State Home Visitation Program Leadership Team meetings and appropriate workgroups.
- Lead ongoing findhelp work as part of the home visiting community engagement strategy to streamline referrals between key community partners and home visiting programs.
- Work with the WVHVP epidemiologist and continuous quality improvement consultants on tracking social determinants of health outcomes and community engagement activities.
- Provide training to new community partners on findhelp and social determinants of health screening impact.
III. EDUCATION:
Training: Bachelor’s degree and background in early childhood and public health preferred.
Substitution: Training: Bachelor’s degree in social work, sociology, psychology, counseling, criminal justice, behavioral science, interpersonal communications, human services, education, special education, elementary education, or secondary education from an accredited college or university may be substituted for the degree in early childhood and public health or current West Virginia social work license.
