Job Title: Referral Coordinator
Job Location: 530 W. Webb Ave. Burlington, NC 27217
Reports to: Manager of Care Transformation
Job Type: Full-Time (Monday-Friday 8a-5p)
Job Description:
The Referral Coordinator is a professional member of our Care Team held accountable for the efficient and effective management of referral tasks. This employee will generally “specialize” in the specific area of referrals. However, this employee will be responsible for all duties listed below. The Referral Coordinator will complete referrals using accurate documentation in accordance with office policies and procedures.
Referral Coordinator Major Duties / Critical Tasks
- Initiation of referrals
- Accurate documentation
- Electronic faxing
- Positive communication with patients, providers, and facilities
- Efficient follow up on referrals
- Cross training of Care Team duties
- Additional Quality Initiatives projects (PRN)

- Knowledge and understanding of referral policy and procedures
- Detailed oriented
- Excellent time management skills
- Flexibility to manage several different job tasks
- Ability to plan, organize, and coordinate personal workflow

Physical Requirements:
- Sedentary Work:
Exerting up to 10 pounds of force occasionally (up to 1/3 of the time) and/or a negligible Amount of force frequently (1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
- Hearing/Vision:
Hear and differentiate low volume sounds to make judgments regarding actions needed, i.e. hear alarm bells, verbal conversations, telephone voices and normal volume sounds.
Close visual acuity is required (clear vision at 20 inches or less and ability to identify and distinguish colors) to perform activities such as preparing and analyzing data and figures, transcribing, viewing a computer terminal, and extensive reading.
