Burlington Health and Rehabiliation Center is seeking a Social Service Assistant to join our social services department. As a Social Service Assistant, you will work closely with our residents to ensure quality care is provided and improve the lives of our residents. This is a full time position working Monday - Friday day hours.Some duties may include:Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needsIdentifying individual social and emotional needsParticipating in the planning of the resident’s admission, community re-entry or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentWork with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stayWork with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communitJob Qualifications:Experience in long term care or SNFExperience working with a diverse aging populationSocial work setting experience is preferredWhy Burlington?Become a part of a Team that enthusiastically supports, recognizes and rewards our team of caring professionals to ensure the best possible outcomes for those entrusted to our care.
