JOB DESCRIPTION -Medical Group Home-Small Setting
LPN, RESIDENTIAL HOMES
Qualifications: Graduate of an approved Licensed Nursing Program; current Tennessee license required. BLS Health Care Provider CPR required. Must complete all required Department of Intellectual and Developmental (DIDD) training as applicable to the position.
Experience: Minimum one year clinically relevant experience and experience working with elderly and/or ID/DD developmental disabilities preferred.
Technical Job Must have knowledge of and technical skills of medical nursing procedures such as tube feeding administration, respiratory treatments, wound care, etc., utilizing basic principles of nursing. Demonstrates the ability to effectively organize, utilize time well, possess strong initiative and work independently.
Must possess or gain knowledge of Intellectual Disabilities, seizure disorders, Autism and other associated diagnoses pertaining to the field of Developmental Disabilities and/or Geriatrics.
Must possess good documentation skills, good communication and professional contact skills, ability to communicate effectively orally and in writing.
Health must be adequate, both mentally and physically to allow for efficiency
accomplishment of assigned duties and responsibilities. Current TB skin testing or statement of “free from communicable diseases.” Must be able to physically lift, adhering to all established lifting protocols.
Supervised clinically by the Director of Nursing and ultimately responsible to the CEO Director
: Usual hours are 12 hours shift rotations but must be available at other timeframes as needed.
LPN, RESIDENTIAL HOMES
Ø Supports the goals of the company AND the person served, as an active team member.
Ø Contributes to and actively participates in the planning and actualization of daily care.
Ø Works closely with and collaborates with the Director of Healthcare, Specialists, Program Coordinators, Team Leaders and other staff, parents, Conservators, physicians, Nutritionists, Independent Support Coordinators (ISC’s), State personnel, Therapists, and other professionals.
LPN, RESIDENTIAL HOMES
Ø Keeps the Healthcare Director, CEO and Direct Support Staff (DSP’s) well informed regarding health-related problems of the persons served; discusses reason for treatments, medications, etc.
Ø Assists in educating all staff so they may better understand the conditions of the person served.
Ø Actively participates in the education of yourself and other staff members.
Ø Maintains ongoing open communication between self and the Director of Healthcare Services and Agency CEO. At all times, maintains contact regarding health related issues or concerns with Agency RN Director and Health Coordinator over the home.
Ø Upon request of the Director participates in training on medically related issues (i.e. infection control, seizure care and protocols, medication effects, individual diagnosis, etc)
Ø Accurately and consistently record all assessments/findings on appropriate Assessment forms and any accompanying forms of documentation as required by the Agency. May be required to electronically complete documentation.
Ø Contributes health-related input to the ISP (Individual Support Plan) as indicated and upon request.
Ø Completes seizure report forms, as applicable to the patient served.
Ø Serves as the Liaison nurse, assisting in making nursing decisions and judgments and keeping supervisor well informed.
Ø Demonstrates knowledge of and utilizes infection control procedures.
Ø Attends physician appointments with the patient as needed, communicating medical information and current status of the patient to the physician. (Appointments shall be scheduled through ProLex, with notification to the home nurse).
Ø Reviews and implements all physician orders as written and follows all Plans of Care.
Ø Provides all treatments per MD order.
Ø Administers all medications to clients in the assigned home per MD order and route.
Ø File and maintain all physician orders and other pertinent related health documents in the person’s home chart as they come in from the ProLex office.
Ø Complies with all agency medication practices and health care polices.
Ø Review the quality and quantity of health-related documentation.
Ø Complete medically related incident reports, when applicable.
Ø Charts appropriately on assigned shift, with no gaps or missing info on the nurse note.
LPN RESIDENTIAL HOMES
Ø Returns all discontinued medications or expired drugs to the office for disposal.
Ø Attends appropriate inservices and staff meetings as directed.
Ø Becomes familiar with and complies with State and Federal regulations as they pertain to the job position and work environment.
Ø Cooperates with State survey teams and other State officials as requested and as applicable.
Ø In a timely manner (immediately upon occurrence), reports concerns of client abuse, neglect, mistreatment and exploitation to the Agency Director, according to protocol.
Ø Remains cognizant of person’s health care needs. Uses medical protocol for decision-making. Reports health concerns to the Supervisor.
Ø Conducts shift to shift monitoring and recording of pertinent assessment data such as weights, vital signs, etc., as assigned.
Ø Assists in the gathering of data for the annual risk assessment upon request from RN.
Ø Assists in monthly QA checks of the residential home at request of RN.
Ø Assists Direct Support Staff in maintaining cleanliness and organizational of the home environment, so as to make it a “home-like” atmosphere conducive to quality of health and life.
Ø Works collaboratively with the Direct Support staff in providing care, such as diapering, bathing, etc., performing all care --together as a unit.
Ø Goes on community outings with the client, assists staff in appropriate preparation and departure via completion of Wheel Chair tie-down equipment in the van.
Ø Reports all abnormal client conditions to the on-call nurse.
Ø Any other duties as may be assigned.
Pay: $23.00 - $25.00 per hour
