What are the responsibilities and job description for the Social Work Case Manager Inpatient, Full-time position at Burke Rehabilitation Hospital?
Responsibilities: Performs 100% intake and psycho-social assessment on all patients and their families. Coordinates services and offers individual, family or group counseling and discharge planning. Acts as primary contact with third party payers and their representatives.
Requirements:
- Must have a Masters degree in Social Work and be licensed to practice in New York, (LMSW) or (LCSW) required .
- Relevant work experience in a hospital or nursing home setting .
- Must be able to able to work 1 Saturday every 6-8 weeks.
- Work Schedule, 40 Hours: Monday- Friday, 8:00am - 4:30pm
*Supervision hours offered to obtain LCSW*
