Role Description
We are seeking a dedicated and compassionate full-time Case Manager to join our team. This on-site position is located in Phoenix, AZ. The Case Manager will be responsible for providing support and guidance to adoptive and birth families, managing case files, coordinating between parties, and ensuring compliance with agency and legal requirements. Duties include conducting home visits, preparing documentation, managing outreach efforts, and serving as a liaison with community resources to assist families and children.

- Strong knowledge of case management, family counseling, and social work practices.
- Excellent communication, active listening, and interpersonal skills.
- Proficiency in maintaining accurate records and preparing detailed reports.
- Ability to work collaboratively with diverse families and navigate sensitive situations with empathy and professionalism.
- Time management and organizational skills to prioritize caseloads effectively.
- Proficiency in Microsoft Office Suite and familiarity with other relevant administrative tools.
- Flexibility to work occasional evenings or weekends as needed to meet clients' needs.
- Bachelor’s degree in Social Work, Human Services, or a related field (Master’s degree preferred).
- Previous experience in adoption services or a similar field is an asset.
- A valid driver's license and reliable transportation for home visits are required.
