Job Summary:
The Housing Navigator is responsible for supporting eligible Medicaid members in securing and maintaining safe, affordable housing under the New York State 1115 Waiver Social Care Network. As part of the Buffalo Urban League’s role within the Western New York Integrated Care Collaborative (WNYICC), the Housing Navigator works closely with Social Care Navigators, health plans, and community housing providers to remove barriers, facilitate housing placement, and ensure successful transitions into stable living environments.
This position is essential to advancing BUL’s mission of empowering individuals and families, reducing barriers to care, and promoting self-sufficiency throughout Erie and Niagara Counties.
Qualifications
Key Responsibilities:
- Accept referrals from Social Care Navigators, screeners and verify client eligibility.
- Conduct housing needs assessments and complete intake documentation.
- Assist clients with housing searches, applications, landlord engagement, and inspections.
- Support clients with assessments (e.g., VI-SPDAT, Vulnerability Index – Service Prioritization Decision Assistance Tool) and referrals through SPOAs when appropriate.
- Provide move-in coordination and support successful transition into housing.
- Help clients set up utilities, apply for HEAP, and connect with community supports.
- Identify barriers to housing stability and provide follow-up support.
- Assist with accessing financial supports for rent, deposits, utilities, moving, and setup costs within program guidelines.
- Maintain accurate and timely records of screenings, referrals, services, and outcomes using the WNYICC data platform.
- Complete case documentation within required timeframes.
- Build partnerships with landlords, housing providers, and community-based organizations.
- Represent BUL and the WNYICC Social Care Network at housing-related events and outreach activities.
Qualifications:
Required:
- Associate degree in related field plus 2 years’ experience, or a Bachelor’s degree with relevant experience in human services, housing, or case management.
- Experience in housing navigation, service coordination, or case management with vulnerable populations.
- Knowledge of local housing resources, subsidies, and public assistance programs.
- Strong verbal and written communication skills; ability to work effectively with diverse populations.
- Strong organizational and documentation skills; attention to detail.
- Reliable transportation and flexibility to travel within Erie and Niagara Counties.
- Ability to work evenings or weekends as needed.
Salary : $23 - $25
