Position Summary
We are seeking a dedicated and experienced Community Care Specialist to join our team in Buffalo, NY. In this vital role, you will provide direct support and advocacy to clients in the community to help them meet daily needs and connect with resources, ensuring they receive the highest level of support to live safely and comfortably at home.

Responsibilities
· Conduct routine visits to clients’ homes.
· Provide case management services including screening, problem identification, prioritization and resolution, care coordination information, and education.
· Document and communicate findings and recommendations to the care team.
· Maintain a positive, supportive relationship with clients and their families.
· Collaborate with caregivers and the interdisciplinary team to implement and update individualized care plans.
· Maintain accurate and up-to-date records and documentation.
· Ensure compliance with all healthcare regulations, policies, and professional standards.
· Some evening and weekend phone availability.

Qualifications
· Bachelor’s degree in Social Work, Public Health, Psychology, Sociology, Human Services, or related field (preferred).
· 1–2 years of experience in community-based case management, care coordination, community health work, or homecare programs (required).
· Strong knowledge of Medicaid services and compliance requirements.
· Strong knowledge of local social support programs.
· Excellent communication, interpersonal, and organizational abilities.
· Ability to manage multiple cases.
· Ability to self-direct and work independently with minimal supervision.
· Ability to collaborate effectively within a multidisciplinary team.
· Proficiency with Microsoft Office and Excel.
· Valid driver’s license and reliable transportation.
