Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in assisted living, nursing homes, and hospital settings, with a strong focus on patient care. This role involves working with individuals facing various challenges, including PTSD, and utilizing effective therapeutic techniques such as motivational interviewing and applied behavior analysis. The Social Worker will play a crucial role in supporting clients through their emotional and psychological needs while collaborating with other healthcare professionals to provide comprehensive care.
Responsibilities
- Conduct assessments to identify clients' needs and develop appropriate care plans.
- Provide counseling and support to individuals dealing with trauma, PTSD, and other mental health issues.
- Collaborate with healthcare teams to ensure holistic patient care in assisted living facilities, nursing homes, and hospitals.
- Implement therapeutic interventions using motivational interviewing techniques.
- Advocate for clients' rights and access to necessary services and resources.
- Maintain accurate documentation of client progress and treatment plans within established systems.
- Facilitate group therapy sessions and support groups as needed.
- Engage with families to provide education on coping strategies and resources available for their loved ones.
Qualifications
- Bachelor’s or Master’s degree in Social Work or a related field.
- Experience in assisted living, nursing home, or hospital settings preferred.
- Knowledge of PTSD care practices and applied behavior analysis techniques is a plus.
- Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team environment.
- Proficiency in utilizing electronic health record systems for documentation purposes.
