Position Summary:
Coordinates and manages the safe and efficient discharge of patients in accordance with Federal, State, and local standards, guidelines and regulations, our established policies and procedures, from skilled nursing to their next phase of care, which may include home care, rehabilitation, or long-term care facilities. Working closely with medical staff, patients, and their families, the Discharge Planner ensures that each patient’s medical and psychosocial needs are met post-discharge. The position requires a fine balance of clinical knowledge, coordination skills, and compassionate communication.
Responsibilities:
• Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations, on changes in policies and procedures to the Administrator.
• Coordinate and manage patient discharge planning activities
• Meets with patient within 72 hours of admission to understand the patient's home needs and discharge plan
• Meets with patient within 72 hours prior to discharge to review and confirm established discharge plan
• Collaborate with healthcare team members including physicians, nurses, and social workers to obtain all pertinent information on patients' needs and develops discharge plans for skilled nursing care
• Arrange for home healthcare services or equipment as required
• Provide patients and families with information on discharge plans and available resources
• Ensure all necessary paperwork and documentation are completed accurately
• Facilitate patient referrals to external healthcare facilities or community services
• Monitor patient progress and modify discharge plans as necessary
• Communication with patient and family throughout the discharge process to ensure needs related to a safe discharge has been met
• Flexibility and adaptability to navigate complex healthcare systems and adjust discharge plans based on patients' changing needs.
• Educate patients and families on follow-up care and medication instructions • Advocate for patient needs and preferences throughout the discharge process
• Other duties as assigned by the Supervisor.
