Job Description:

**

Ensure physical documentation is accounted for according to Medicaid/Medicare and organizational guidelines to facilitate payment for services.

Job Duties/Responsibilities/Functions (including but not limited to)

• Locates and retrieves records to accurately audit and verify patient information.
• Updates client charts with information in a timely manner.
• Audits charts, records, and record documentation deficiencies for attending physicians to correct.
• Track location of charts using appropriate software.
• Respond to information requests either in person or by telephone in a timely manner.
• As requested, locate and make ready charts or information from charts in a timely manner.
• Performs other duties as assigned.

Minimum Qualifications:

• High school diploma

CERTIFICATIONS/SKILLS: Basic office software

MINIMUM EXPERIENCE: One year of experience

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently or a negligible amount constantly to move objects.
