We are hiring a new Navigation Manager to join our team. As the Navigation Manager you will play a critical role in bridging hospital discharge and community-based care for individuals with behavioral health and substance use challenges. This position focuses on system navigation and care coordination by supervising System Navigators, managing services, and maintaining effective communication among clients, families, and community resources.

Utilizing a "closed loop referral" approach, the Navigation Manager ensures that referrals are effectively tracked and completed to reduce barriers to recovery and promote wellness. The role oversees the development and implementation of person-centered care plans that address both clinical needs and social determinants of health. It also involves supervision, performance monitoring, and strategic planning to align with Buckelew Programs. By fostering a culture of client empowerment and community integration, the Navigation Manager helps reduce hospital re-admissions, improve care coordination, and ensure timely access to appropriate levels of care.

RESPONSIBILITIES:

Administrative

  • Manage day-to-day operations of the STPH Navigation Program to ensure smooth, goal-aligned functioning.
  • Develop and refine workflows, documentation processes, and procedures to support seamless care coordination and the "no wrong door" model.
  • Monitor and manage program budgets in alignment with organizational and financial goals.
  • Track and report key performance metrics, including outreach, service engagement, referral completion, and client outcomes.
  • Serve as the primary liaison between internal teams and external partners to ensure timely and effective communication.

Staff Supervision and Development

  • Supervise System Navigators, including recruitment, onboarding, training, and performance management.
  • Lead regular team meetings, individual supervision sessions, and case consultations to ensure staff engagement and quality care delivery.
  • Support staff development through ongoing training on navigation strategies, compliance standards, and best practices in care coordination.
  • Coordinate staff schedules to ensure adequate coverage and meet program needs.

Clinical Oversight

  • Provide oversight and guidance to staff in planning, delivering, and documenting client services.
  • Support the development and review of client-centered care plans that address both clinical and non-clinical (e.g., housing, food insecurity) needs.
  • Guide staff in managing complex cases, including client engagement strategies and addressing barriers to care.
  • Monitor and support the "closed loop referral" process to ensure referrals are timely and completed.
  • Provide culturally responsive care, accommodating language preferences and individual needs.
  • Offer direct services, crisis intervention, and clinical support when needed.
  • Ensure compliance with CalAIM requirements in service documentation and delivery.

Program Quality and Compliance

  • Ensure all program activities are compliant with regulatory, contractual, and organizational standards, including CalAIM Enhanced Care Management and Community Supports.
  • Conduct or assist with regular audits of program documentation and service delivery.
  • Support and oversee quality assurance initiatives, including care plan reviews and outcome tracking.
  • Contribute to continuous quality improvement through data evaluation, reporting, and stakeholder feedback.
  • Uphold client confidentiality, privacy standards, and agency compliance protocols.

Community Engagement and Outreach

  • Represent Buckelew Programs in community forums, coalitions, and public events to advocate for client needs and promote services.
  • Cultivate and maintain partnerships with hospitals, behavioral health providers, housing agencies, and other community organizations to strengthen referral pathways.
  • Develop outreach strategies to enhance program visibility and connect with clients, including responding to Partnership Health-generated outreach lists.
  • Maintain up-to-date knowledge of community resources and ensure these are shared with staff, clients, peers, and families.
  • Advocate for client and family interests on relevant committees and community task forces.
Qualifications:

Education and Experience:

  • Bachelor’s or Masters degree in Social Work, Psychology, Public Health, or a related field.
  • Relevant certifications such as Certified Drug and Alcohol Counselor, Psychiatric Technician, RN license, Peer Support Specialist, Patient Navigation Certification, or Community Health Worker certification preferred.
  • Minimum of 5 years’ experience in a behavioral health or healthcare setting.
  • 5 years of supervisory or team leadership experience required.
  • Strong understanding of family and group dynamics, crisis management, treatment planning, and documentation.
  • Experience with electronic health records (EHR) systems (2 years preferred).
  • Experience in a first responder or crisis response field is a plus.

Professional Skills:

  • Fluent in English; bilingual (English/Spanish) preferred.
  • Strong organizational, interpersonal, and communication skills.
  • Able to work independently with sound judgment and professional boundaries.
  • Flexible, dependable, and receptive to feedback and supervision.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook) at intermediate to advanced level.
  • Strong ability to manage multiple projects simultaneously.
  • Commitment to working with underserved populations and supporting recovery-oriented care.
  • Must have a valid California driver’s license, an insurable driving record, and access to an insured personal vehicle for work-related travel.
  • Must successfully pass a Live Scan background check.

Salary : $80,000 - $85,000
