Role Description

This is a full-time, on-site role for a Physical Therapist located in Merced, CA, where we specialize in orthopedics, sports physical therapy, and rehabilitation. The Physical Therapist will be responsible for evaluating and treating patients with physical therapy techniques, developing individualized treatment plans, and providing rehabilitation services. Day-to-day tasks include assessing patient needs, designing therapy programs, tracking progress, and collaborating with other healthcare professionals. The role requires a focus on delivering high-quality patient care and ensuring optimal recovery outcomes.


Qualifications:

  • Doctorate in Physical Therapy
  • California PT License
  • CPR Certification


Duties:

  • Responsible for initial evaluations and reassessments/treatment of patients on the caseload for their follow-ups, progress notes and discharge summaries.
  • Complete documentation of progress notes and flow sheets/charts according to guidelines of the department in a timely manner.
  • Understand an inclusive knowledge of insurance payors, including private insurances, Central California Alliance for Health, Medicare, Worker’s Compensation, etc.
  • Follow all state and national regulations to maintain an active credential including the completion of all continuing education and ethics requirements.
  • Establish an individualized plan of care for each patient and properly executing, modifying, and monitoring all aspects of patient care using PT Aides and clinical staff.
  • Provides skilled physical therapy treatment plans in accordance with physician orders and by assessing and interpreting evaluations and test results of patients.
  • Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, recommending and/or providing assistive equipment; recommending follow-up programs.
  • Promptly and thoroughly documents and maintains clinical and administrative records regarding patient limitations/interventions, treatments, responses and progress to administered therapy as well as discharge summaries.
  • Records treatment charges daily and completes billing in accurate and timely manner.
  • Supervises and directs Physical Therapy Assistants and Physical Therapy Aides in direct patient care and related activities as requested.
  • Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.
  • Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.
