What are the responsibilities and job description for the PHARMACIST position at Brunswick Hospital Center?
It is the policy of Brunswick Hospital Center to provide the highest quality of pharmaceutical care. Pharmacists must be licensed and registered in NY State with impeccable records of performance.
PROCEDURE:
I. The Staff Pharmacist shall be responsible for:
- The dispensing of drugs, chemicals, and pharmaceutical preparations.
- The filling and labeling of all drug containers issued to services to which medication is to be administered.
- The inspection of all pharmaceutical supplies on all services.
- The maintenance of an approved stock of antidotes and other emergency Drugs.
- The dispensing of all narcotic drugs and the maintenance of their perpetual inventory.
- The specifications both as to quality and source for the purchase of all drugs in conjunction with central bids.
- The provision of information concerning medications to physicians, interns and nurses.
- The charging for drugs and pharmaceutical supplies to patients and the reconciliation of billing.
- The maintenance of the confidentiality of all records as they pertain to the patient.
- The maintenance of adequate control over the requisitioning and dispensing of all drugs and pharmaceutical supplies.
- Adherence to pharmacy policies and procedures in accordance with the established policies of the hospital that guide and support the provision of services.
- The maintenance of the facilities of the department.
- Participation in teaching programs to ancillary professionals and patients.
- Involved in maintaining a pharmacy Quality Improvement program.
- Assisting with the implementation of the decisions of the Pharmacy & Therapeutics Committee.
- Assisting with the preparation of periodic reports on the progress of the department for submission to the administration of the hospital.
- To maintain State and Federal required records to maintain adequate accountability and control.
- Responds and resolves issues identified regarding patient satisfaction.
- Monthly Narcotic Inventory of Pyxis med stations as assigned by the Director.
- Daily Narcotic Inventory and maintenance of records of received inventory, expired inventory, and returned inventory, including any drug recalls of controlled substances.
- Weekly Review/Emptying of assigned Pyxis Med station External Return Bins.
- Weekly Review/Removal of outdated medications in Pyxis Med stations as per policy.
- The pharmacist will be cross-trained and able to perform the following tasks:
- Antibiotic Stewardship Review and Reporting.
- Clozapine REMS maintenance, reporting and follow-up of labs.
- Daily Reconciliation of Billing for Discharged patients.
- Regularly maintains information in the Pyxis Console including that related to patient information, formulary, and all processes of Automated Dispensing.
II. Job Requirements:
A. Education and Formal Training Required:
Graduate from an accredited College of Pharmacy. Licensed to practice in pharmacy in New York State. Through training and/or experience, possess necessary knowledge to provide oversight of medication regimens for the patient populations treated in the facility including knowledge of dosing for specific age populations.
B. Experience:
A minimum of three years of hospital experience is desirable.
C. Contacts Required:
Frequent and professional communication with physicians, nurses, patients, and other professional personnel. Regularly work with confidential information.
III. General Statement of Duties:
In addition to the on-site activity, the staff pharmacist will be available 24 hours a day, seven days a week via home phone or beeper, as required. He/she may assist in the teaching of students and house staff where applicable. The Staff Pharmacist devotes an adequate amount of time to assist the Director of Pharmacy in administrative activities. Examples:
A. Assumes responsibility for pharmacy services, policies and procedures, and their administration in accordance with established policies of the institution and in the absence of the director.
B. Assumes overall supervision of the pharmacy service staff and is responsible for all drugs and distribution in the institution, as required.
C. Maintains adequate continuing education regarding competency.
D. Provides in-depth pharmacologic and pharmacokinetic consults (verbal or written) when requested or when appropriate.
E. Provides formal in-service programs for the Nursing Department on a yearly basis as requested by the Director of Pharmacy.
F. Assist in coordinating all pharmacy activities with other ancillary professional departments.
G. Assist in responsibility for the department's continuous, effective operation and for the improvement of its performance.
H. Orients and provides in-service training and education to all persons in the department.
IV. Judgment and Initiative Required:
Must exercise sound administrative and professional judgment using initiative consistent with a high level of patient care. Requires the ability to work efficiently under broad guidelines and must frequently exercise his or her own judgment in decision making.
- The pharmacist will exhibit at all times while on duty an attitude of cooperation, teamwork, and courtesy in all interactions with other staff as well as patients and family of patients.
- The pharmacist will employ politeness and telephone etiquette with all staff, patients, pharmacies, outside vendors and families who call the pharmacy seeking information and/or assistance.
- The pharmacist will direct all inquiries and/or requests to the director of pharmacy for any HIPAA-related concerns.
V. Accuracy Required:
Requires a very high degree of accuracy.
VI. Responsibility for Work of Others:
Safety of patients and department personnel under his or her supervision.
VII. Responsibility for Equipment and Supplies:
Totally responsible for all the equipment, supplies and staff in his or her department.
VIII. Mental Demand:
Diversified job requiring alertness and ability to handle a heavy volume of work. Attention to details over an extended period of time expected.
IX. Emotional Demand:
Must be emotionally stable. Stress and strain in meeting demands encountered in patient care and in administrative duties.
X. Orientation:
The Staff Pharmacist is oriented to the specialized functions of the hospital, including types of patients served and specialized medication needs of these patients.
XI. Evaluation:
The Director of Pharmacy will perform an evaluation consistent with the job description of each staff pharmacist and technician at least annually.
XII. Other:
As the pharmacist is available only a limited number of hours each day, CPR training is not a requirement of pharmacy staff.
