Brownsburg Health Care Center is seeking an experienced Social Services Director to lead psychosocial services in our Skilled Nursing Facility (SNF). Key Responsibilities
- Complete comprehensive psychosocial assessments for residents
- Develop and implement individualized care plans
- Provide emotional, social, and mental health support to residents and families
- Lead discharge planning, transitions of care, and community resource referrals
- Advocate for resident rights, dignity, and quality of life
- Collaborate with nursing, therapy, and interdisciplinary care teams
- Maintain accurate documentation in compliance with state and federal regulations
- Participate in state surveys, audits, and compliance reviews
- Supervise or support social services staff, as applicable
- Bachelor’s degree in Social Work or related Human Services field (Psychology, Sociology, Human Services, Rehabilitation Counseling)
- Licensed, certified, or registered to practice in Indiana
- Minimum 1 year of supervised social work experience in a healthcare setting
- Knowledge of CMS regulations, resident rights, and discharge planning processes
- 2 years of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) environment
- Previous Social Services Director or leadership experience
