Duties
- Supervise and coordinate staff to deliver comprehensive patient care within the consumers home.
- Oversee patients by completing home visits every 60 days to report significant changes or revision of the care plans.
- Collaborate with interdisciplinary teams including physicians, case managers, and specialists in areas such as dementia care, hospice care, acute pain management, or emergency medicine to optimize patient outcomes.
- Provide training and mentorship to nursing staff on topics like basic life support (BLS)/CPR
- Participate in discharge planning, patient education, and family communication to promote continuity of care and patient satisfaction.Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Strong background in medical nursing, geriatrics, or home & community care environments.
- Experience working with individuals with disabilities or developmental disabilities is preferred.
- Demonstrated leadership skills with the ability to supervise staff effectively while maintaining high standards of patient care.
