Summary:
GRANT FUNDED - Provides screenings and assessments to women and their families eligible for the Healthy Start program within their homes. Evaluates individual patient needs to develop tailored care plans and coordinates referrals for services aimed at addressing social determinants of health. This approach is designed to improve maternal and infant outcomes by ensuring comprehensive support and resource allocation. Spanish or Haitian Creole preferred. New Grads welcomed.
Grant funded positionsat Broward Health are often temporary or project-based, as they depend on the duration of the grant funding. Once the grant period ends or the allocated funds are exhausted, the position may be terminated unless additional funding is secured.
Education: Bachelor's degree in Social Work
Credentials: Bilingual preferred; Spanish or Creole
This position will require successful completion of a Level II Background Screening
