We are seeking a compassionate and clinically driven Nurse Practitioner to join a collaborative nephrology team, providing comprehensive care to dialysis patients across multiple outpatient dialysis centers.
🔹 Perform routine patient rounds Monday through Friday across:
- Two primary dialysis units (approximately 50 patients each)
- Additional nearby units contributing to a total census of ~140 patients ( including the big ones)
🔹 Assess fluid status, labs, medications, and treatment plans
🔹 Collaborate closely with Nephrologists and Dialysis Teams
🔹 Maintain accurate EMR documentation
🔹 Deliver high-quality, patient-centered care
✅ Active Nurse Practitioner License
✅ Nephrology, Dialysis, Internal Medicine, or Primary Care experience preferred
✅ Strong clinical assessment and communication skills
