What You’ll Do:

• Provide and evaluate direct nursing care for residents

• Serve as Charge Nurse, supervising PCAs

• Support personal hygiene, nutrition, mobility, and restorative care needs

• Transcribe physician’s orders and assist with medical rounds

• Observe and report resident condition changes to RN Supervisor or physician

• Assist with care plans and coordinate multidisciplinary care

• Administer medications safely, including controlled substances

• Order, document, and manage medications and lab tests as needed

• Arrange transportation for resident appointments

• Ensure comfort and safety measures are applied consistently

• Accurately document and communicate resident care activities

• Current New York State LPN license required

• Previous nursing experience in assisted living, long-term care, or related setting preferred

• Strong clinical assessment and documentation skills

• Leadership ability to supervise and support PCAs
