The Public Health Department is seeking a compassionate, organized, and resourceful Early Intervention Service Coordinator to support infants and toddlers with developmental delays or disabilities and their families. This professional position is responsible for coordinating services for children from birth to thirty-six months of age, assisting families in accessing Early Intervention services, and facilitating the development and implementation of Individualized Family Service Plans (IFSPs).
The Early Intervention Service Coordinator serves as a liaison between families, evaluators, service providers, and community agencies to ensure children receive appropriate and timely services that meet both child and family needs.
Key Responsibilities- Contact families of developmentally delayed or disabled children to explain the Early Intervention Program and family rights
- Utilize the New York State Early Intervention System (NYEIS) to input and manage client information and communicate with providers
- Gather information regarding child and family needs to prepare social summaries
- Assist families in identifying appropriate evaluators and service providers
- Coordinate and facilitate the development, review, and evaluation of Individualized Family Service Plans (IFSPs)
- Monitor services to ensure timely delivery, quality of care, and family satisfaction
- Coordinate communication among service providers, agencies, and families
- Develop transition plans for children exiting the Early Intervention Program
- Maintain accurate records, reports, and case documentation
One of the following is required:
- Bachelor’s Degree in Education, Psychology, Sociology, Nursing, or a closely related field; OR
- Bachelor’s Degree in any field and one year of experience in case management, nursing, social work, or teaching; OR
- An equivalent combination of training and experience
- Knowledge of Early Intervention programs, regulations, and procedures
- Understanding of child development and special needs services
- Familiarity with laws and regulations related to children with disabilities
- Strong interpersonal, communication, and organizational skills
- Ability to manage sensitive situations with professionalism and tact
- Proficiency with computer systems and online documentation platforms
- Ability to maintain accurate and confidential records
- Strong problem-solving and relationship-building abilities
This position involves regular interaction with families, providers, and community agencies. Field work and travel within the service area may be required. Candidates must have reliable access to transportation to meet job responsibilities in a timely manner.
