What are the responsibilities and job description for the Medical Case Manager position at Brooklyn Hospital Center?
The Medical Case Manager ensures that all PATH adult program patients are referred to appropriate health and psychosocial services, as well as for entitlements.
Qualifications:
- High School or GED
- Associate or Bachelor’s Degree – Highly Preferred
- At least two (2) years of experience in HIV treatment, medical case management and/or health education
- Must have strong written and oral communication skills.
- Prior experience using an Electronic Medical Record preferred.
- Bilingual English/Spanish or English/Creole preferred
