Position Summary:
Responsible for providing comprehensive Patient Experience Manager services to patients in conjunction with clinical counselors and clinical team.
Essential Job Functions:
- Works with patients to develop individualized case management plans.
- Continually monitor and evaluate each patient’s progress through their case management plan and develop corrective action revisions to the plan as needed.
- Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, and County guidelines.
- Assist patients in obtaining necessary resources to achieve case management goals.
- Maintain patient related KIPU data, including case notes.
- Prepare case management related reports including but not limited to outcomes and successes.
- Conducts crisis intervention as needed.
- Communicates and works effectively with a team, consults with the multi-disciplinary team, patient, and referent (consistent with CFR 42 and CFR 45) to address clinical issues both verbally and in writing related to the individualized treatment plan, continuing care, including types of care, level of care.
Knowledge, Education & Experience:
- Master’s degree or higher, in chemical dependency or related field, with a practicum in health or human services agency, preferably within a drug and alcohol setting OR
- Bachelor's Degree with a major in chemical dependency or related field, with a one (1) year of clinical experience in health or human services agency, preferably in a drug and alcohol setting OR
- Associate degree with major in chemical dependency or related field and two (2) years of clinical experience in health or human services agency, preferably in a drug and alcohol setting OR
- Combination of education or business experience OR
- Knowledge of addiction/recovery processes along with related resources.
- Exhibits capacity for developing a therapeutic relationship with patients and their family members.
- Demonstrates familiarity and experience with “core case management functions"'.
- Has knowledge of, and experience in, the facilitation of group process, as well as individual counseling sessions.
- Demonstrates knowledge of patient behavior management techniques.
- Demonstrates an understanding of basic computer skills and a willingness to utilize an electronic medical record (EMR).
