The Rapid Rehousing Program for Survivors of Domestic Violence aims to mitigate homelessness in New York by supporting our clients with transitioning from shelter to permanent housing whilst also supporting small landlords in providing a safe place for survivors of domestic violence. Our Housing Specialists provide comprehensive and timely support in finding a safe and secure housing unit, and our Aftercare Specialists help clients for approximately six months after their placement to become self-sufficient and financially independent.

Qualifications
- Bachelor’s degree
- Past aftercare case management experience; with experience supporting survivors of Intimate Partner Violence and Domestic Violence ; preferred
- Proficiency in English required; proficiency in a second language preferred.
- Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment.
- Assist in identifying, securing, and maintaining safe and stable housing
- Liaise with shelter providers to assist sheltered clients with housing services
- Provide direct assistance with housing applications, subsidies (e.g., vouchers), and required documentation
- Provide one on one case management services to building/neighborhood residents.
- Establish and maintain relationships with landlords, property managers, and community housing resources
- Provide aftercare case management to housed survivors for 6-9 months
- Plan and facilitate workshops for Small Landlords.
- Participate in all mandatory agency trainings.
- Attend internal and external events.
- Submit monthly reports to supervisor.
- Perform additional duties as assigned
- The case manager will be trauma informed and provide trauma-informed case management care to
- The case manager will carry a caseload of aftercare clients providing home visits once monthly
- The case manager will direct the client to the appropriate service or program and will complete a prescreening/intake.
- The case manager will enter information into databases confirming whether the referral was accepted
- The case manager will enter case notes to document aftercare visits in AWARDS database concerning missed appointments, declined services, etc.
- The case manager will assist with housing interviews and identifying housing opportunities for potential aftercare clients
- The case manager will continue to enter information regarding the status of the clients’ case.
- The case manager will attend meetings and conduct presentations
- The case manager will assist the Program Director writing reports to funders
- The case manager will conduct ongoing monthly or bi-monthly home visits with staff
- All other duties as requested by the supervisor
- Excellent communication, writing and interpersonal skills
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds
- Ability to stand, walk, or sit for long periods of time
- Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
- Ability to bend and retrieve objects and/or documents
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or fieldwork
