The BronxWorks Homeless Outreach Team (HOT) is a mobile unit that works with homeless individuals on the streets, 24 hours a day, 365 days a year. Since 1995, we have helped street homeless people obtain appropriate services, move to safer environments, and get placed in permanent or long-term transitional housing. The team is composed of outreach workers, case managers and social workers and also partners with psychiatric and medical providers to connect clients with appropriate care.

Responsibilities
- Independently manage a caseload
- Conduct comprehensive intake assessments
- Conduct regular field visits to engage and build rapport with chronically homeless individuals
- Work with clients to develop goals and service plans
- Provide referrals and direct assistance to support clients applying and/or attending appointments for: mental health services, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing
- Document all interactions with or on behalf of clients; monitor and document client progress toward service plan goals
- Maintain familiarity with social services and resources available both on-site and in the community
- Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required
- Attend regular supervision sessions with direct supervisor
- Drive agency vehicle to and from client locations and appointments
- Perform additional duties as assigned
- Bachelor’s degree in social work or a related field of study
- Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points
- One-year of case management or related work experience preferred
- Ability to engage and build rapport with populations with high support needs
- Strong oral and written communication skills
- Detail-oriented and resourceful with strong time management and organizational skills
- Proficiency in Microsoft Office suite and other standard business technology
- Bilingual/multilingual preferred, but not required
- Ability to use a computer for prolonged periods
- Ability to occasionally lift and/or move up to 10 pounds
- Ability to stand, walk, or sit for long periods of time
- Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
- Ability to bend and retrieve objects and/or documents
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
- ‘TB Test required with the first 120 days of employment’ for Shelters and Homeless Outreach programs
