Bronx Independent Living Services (BILS) is a nonprofit, community-based organization dedicated to empowering people with disabilities to understand and exercise their civil and human rights and to live fully integrated lives in mainstream society.
We help individuals make informed decisions about their own lives by connecting them with education, skills development, and the right resources. At a systems level, we advocate to remove architectural, communication, and attitudinal barriers that have held the disability community back for far too long.
ROLE OVERVIEW
The Transition Specialist is a key member of BILS MFP/Open Doors program, working directly with individuals living in nursing homes who want to return to the community. You will guide, support, and advocate for participants through every step of the transition process from initial planning at the facility to preparedness for community living at the time of discharge.
You may also support transitions for individuals served by the Office for People with Developmental Disabilities (OPWDD) who are moving into community-based settings.
WHAT YOU’LL DO
• Receive and manage referrals from the Regional Transition Center, ensuring each participant’s transition is fully supported from start to finish.
• Develop and facilitate person-centered transition plans, including securing housing and coordinating community services throughout their stay at the facility and ensure a safe, supportive, and successful transition into the community.
• Coordinate nursing home visits, participate in discharge planning meetings, and collaborate with relevant social service and nursing team.
• Provide clear and supportive guidance to individuals and their families regarding available community resources and services.
• Introduce participants to the peer mentor program and submit referrals when appropriate.
• Work collaboratively with the transition team to identify and address barriers to community transition, including medical and behavioral health needs.
• Maintain accurate, timely documentation in BILS and contracted provider databases.
• Attend BILS in-service trainings, team meetings, and agency activities.
• Perform other program-related duties as assigned.
WHAT WE’RE LOOKING FOR
Education & Experience
• 2 years minimum experience and community-based long-term care, Medicaid programs such as MMC, MLTC, TBI, and NHTD waivers, or human services preferred. Degree in human resources or a related field is a plus.
• Working knowledge in patient care and a strong commitment to person-centered planning and consumer choice.
• Basic knowledge of Medicaid, Medicare, eligibility, and coverage.
• Experience with NHTD, TBI, and/or OPWDD waivers (preferred)
• Background in service coordination, care management, or case management (preferred)
• Experience working with individuals with disabilities (preferred)
Skills & Competencies
• Strong written and verbal communication skills with excellent organizational ability
• Ability to actively listen and provide guidance throughout the transition process.
• Knowledge of community and government resources that support transitions from institutional to community living
• Solid time management skills to balance outreach, caseload, documentation, and administrative duties.
• Comfortable working independently and collaboratively as part of a team
• Proficiency with Microsoft Office and general internet research
• Understanding of disability-related issues (preferred)
• Bilingual (preferred)
