Salary Range: $86,000 – $105,000 annually (commensurate with experience)
Sign-On Bonus: $12,000

Your Role And Impact
- Serve as a primary coordinator of all care for hospice patients, ensuring seamless and compassionate end-of-life care.
- Conduct comprehensive assessments and create personalized care plans in collaboration with patients, families, and the interdisciplinary team.
- Provide expert nursing care, symptom management, and emotional support tailored to each patient's needs.
- Act as a liaison, advocating for patients' and families' needs and coordinating with healthcare providers and community resources.
- Educate families and caregivers on end-of-life care, ensuring they feel supported and informed.
