Position Summary:
The Home Care Services Liaison is responsible for building and maintaining relationships with community referral sources. This role serves as a key link between the agency and the community, ensuring awareness of services, supporting growth initiatives, and ensuring smooth transitions for patients onto home care and hospice services.
Key Responsibilities:
- Build and maintain strong relationships with hospitals, medical practices, SNFs, ALFs and other referral sources
- Educate referral sources on available home care and hospice services and admission criteria
- Generate referrals through ongoing outreach, networking, and relationship management
- Conduct regular in-person visits and follow-ups with referral sources
- Collaborate with intake and home care/hospice leadership to ensure smooth patient transitions and admission to services
- Represent the agency at community events, health fairs, and meetings
- Stay informed on industry trends, regulatory changes, and competitor activity
Qualifications:
- 1-3 years of experience in healthcare marketing, home care, hospice or related field.
- Strong knowledge of home care and hospice services and referral processes
- Excellent communication, customer services, and interpersonal skills
- Self-motivated, goal-oriented, and able to work independently
- High school diploma
