What You’ll Do
As a key member of the Interdisciplinary Group, you will work under RN supervision and provide compassionate personal care, including:
- Bathing, grooming, skin care, and hygiene
- Providing emotional support to patients and families
- Assisting with mobility, transfers, and range‑of‑motion exercises
- Taking and recording vital signs
- Light meal prep, feeding, and simple household tasks
- Ensuring a safe, clean, comfortable home environment
- Reporting patient condition changes to the Case Manager
- Supporting families with respite care
- Documenting care accurately and timely
Requirements
- Active CNA certification
- Prefer 1 year of CNA experience (hospice/home health a plus)
- Valid driver’s license and reliable, insured vehicle
- Current CPR certification
- Willingness to complete a background check
