What You’ll Do

As a key member of the Interdisciplinary Group, you will work under RN supervision and provide compassionate personal care, including:

• Bathing, grooming, skin care, and hygiene
• Providing emotional support to patients and families
• Assisting with mobility, transfers, and range‑of‑motion exercises
• Taking and recording vital signs
• Light meal prep, feeding, and simple household tasks
• Ensuring a safe, clean, comfortable home environment
• Reporting patient condition changes to the Case Manager
• Supporting families with respite care
• Documenting care accurately and timely

Requirements

• Active CNA certification
• Prefer 1 year of CNA experience (hospice/home health a plus)
• Valid driver’s license and reliable, insured vehicle
• Current CPR certification
• Willingness to complete a background check
