RN - Home Care Community RN $70,720.00/annual
Schedule: Monday - Friday, 9:00 am - 5:00 pm (1 hour lunch), 35 hour workweek
Position Summary: This position is responsible for conducting screenings and developing care plans in compliance with Executive Office of Aging and Independence (AGE) and agency guidelines.
Principal Accountabilities:
- Assures that home care clients receive high quality nursing support:
- Conducts at least annual assessments of clients receiving Personal Care, Home Health, or Supportive Home Care services.
- Develops and reassesses personal care plans that meet consumer choice and budget guidelines.
- Coordinates with vendors and case managers to ensure that personal care plans are implemented appropriately.
- Participates in interdisciplinary work with case managers; shares clinical information about clients.
- Assures that home care clients receive services necessary to maintain independent living in the community
- Assures that screenings are conducted on a timely basis and in compliance with AGE and agency regulations
- Documents visits, phone calls, and all activity in accordance with AGE and agency requirements; maintains client records accurately and within the mandated time frame
- Participates in office coverage responsibilities on a rotating basis.
Qualifications:
Education and Experience:
- Valid Massachusetts RN license.
- A Bachelor of Science degree in Nursing or Associates of Science degree in Nursing and at least one year of clinical experience and/or nursing in a community or long term care setting; or a Diploma RN and two additional years of community-based nursing experience or long term care direct service experience.

- strong assessment skills,
- ability to multi-task and work in a fast-paced, high volume work environment.
- Driver’s license and reliable transportation required.
