Key Responsibilities
- Conduct comprehensive nutrition assessments upon enrollment, semi and annually, and with any significant change in participant condition.
- Develop, implement, and monitor individualized nutrition care plans.
- Provide medical nutrition therapy (MNT) for chronic diseases.
- Monitor weight trends, laboratory values, oral intake, dentition, and nutritional risk indicators.
- Recommend therapeutic diets, oral nutrition supplements, and texture‑modified diets.
- Actively participate in IDT meetings, care planning, and case conferences.
- Provide individualized nutrition education and counseling to participants and caregivers.
- Conduct home visits as needed to assess food access, safety, and ability to prepare meals.
- Coordinate with home care teams to address nutrition‑related barriers in the community setting.
- Organize home delivered meals and supplements as required.
- Oversee the PACE kitchen area.
- Assist in monitoring meals including intake, diet compliance, and tray accuracy.
Skills, Knowledge & Expertise
- Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
- Minimum of 1–2 years of clinical nutrition experience (geriatrics, long‑term care, or community health preferred).
- Minimum of one (1) year working with the frail and elderly.
- Certified Diabetes Educator (CDE), desired.
- ServSafe Food Protection Manager certification required - if certification is not held at the time of hire, it must be obtained within six months of the hire date.
- Valid driver's license and reliable transportation.
