- The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patient-centered, creative, and collaborative in their approach to care.
- The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a well-supported, team-based environment.
- Participating in daily interdisciplinary team meetings
- Completing periodic health assessments
- Assessing and treating acute needs and illness
- Advanced Care Planning
- Patient and Caregiver Education
- Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan
- Master's degree in nursing or satisfactory completion of a physician assistant program
- Active Registered Nurse (RN) license in the state of Michigan.
- CPR certification or willingness to obtain
- Degree in Nursing; bachelor’s degree in nursing (BSN) preferred
- Minimum of 1 year of nursing experience.
- Previous experience in home health, case management, or a related field preferred.
- Current DEA registration
- State of Michigan Controlled Substance License
- Minimum of 1-year experience with the frail and elderly or long-term care population preferred
