Day-to-day responsibilities include assessing client needs, creating and implementing individualized rehabilitation plans, facilitating skill-building and psychoeducational groups, and providing one-on-one support in areas such as daily living skills, coping skills, and vocational readiness. Qualifications

- Candidates should possess foundational knowledge of Medicine and Physical Therapy to understand treatment plans and collaborate with medical providers.
- Candidates should possess experience or familiarity with Inpatient Care to support transitions between inpatient and community-based services.
- Bachelor’s degree in psychology, social work, rehabilitation counseling, occupational therapy, or a related human services field; equivalent experience may be considered based on state regulations.
- Experience working with individuals with serious mental illness or co-occurring disorders in community or clinical settings.
- Strong skills in assessment, treatment planning, documentation, and interdisciplinary collaboration.
- Effective communication, de-escalation, and crisis intervention abilities, with a commitment to trauma-informed and recovery-oriented care.
