Full Time Family Advocate
Position Summary
The Family Advocate plays a vital role in supporting caregivers and families through advocacy, case management, and connection to community resources. This position uses a strengths-based approach to empower families, promote independence, and ensure access to essential supports. This role requires strong judgment, compassion, and the ability to build meaningful relationships while maintaining professionalism and integrity at all times.
Key Responsibilities
- Provide direct case management and advocacy services to caregivers and families
- Conduct strengths-based assessments and develop individualized goals
- Connect families to community resources (housing, treatment, education, etc.)
- Conduct home visits and meet families in community settings
- Build and maintain relationships with community providers and service teams
- Support caregivers in navigating social service systems
- Coordinate resources to meet basic needs (food, housing, utilities, etc.)
- Maintain accurate and timely documentation in electronic systems
- Collaborate with clinical and program staff to support family success
- Develop and facilitate educational opportunities for families
- Participate in team meetings, trainings, and case conferences
- Transporting clients as needed
Qualifications
Required:
- Bachelor’s Degree in Social Work
- Licensed Social Worker (LSW)
- 1 year experience working with at-risk families and/or substance use populations
- Valid driver’s license, reliable transportation, and insurance
- Ability to pass BCI, FBI, OSAPS, and NSOPW background checks
- An occasional weekend day or evening may be necessary.
Preferred:
- Master’s Degree
- 5 years of relevant experience

Pay: $50,000.00 - $73,000.00 per year
