Brightstone Transitions is a coaching and mentoring program for young men and women, ages 18 to 26. We offer highly individualized programming in order for young adults to develop self-awareness and self-respect. Our dedicated team supports clients in their quest for independent living skills, personal growth, and emotional maturity. Brightstone specializes in creating customized support programs facilitated by mentors in an Independence Through Application approach. This immersive approach allows for learning opportunities in real-life situations. Our young adults move beyond program–centered activities and, with guidance from mentors, they obtain employment, learn the power of volunteerism, enroll in college or technical school, and experience what it means to be truly self-sufficient.
Brightstone Transitions is looking for a confident, organized, and motivated individual to fulfill the role of Case Manager. The Case Manager role is a supervisory role that oversees the coaching and development of a team of employees. This individual is responsible for managing a team of mentors as well as overseeing the care of young adults diagnosed with ASD.
The role of the Case Manager provides a safe and supportive environment for both staff and clients and work with a treatment team consisting of Program Director, Therapist, and Psychiatrist to implement treatment plans and goals. The Case Manager meets with staff at least once per week to provide support and both monitor and document daily and long-term progress toward the treatment goals of the clients. The Case Manager oversees and manages client progress and reviews documents and communicates regularly with families regarding progress. The Case Manager plans and oversees daily activities, workshops, and group meetings.
This position requires the ability to help bring out the strengths of staff and involves creativity in planning activities for both staff and clients. The Case Manager is responsible for modeling healthy and appropriate interactions, providing conflict resolution, and teaching life skills. The Case Manager supports staff in finding "teachable moments" or windows of opportunity to teach life skills, self-awareness and other concepts to clients with ASD.
Required Qualifications: Master’s degree in psychology or other social science field or a Bachelor's degree and at least five years of relevant experience. Background in working professionally with young adult clients with either ASD and/or a diversity of issues including mental health, family dysfunction, lack of motivation, and learning disabilities strongly preferred. Related experience will be considered. Ability to supervise, enforce boundaries, and provide role modeling and leadership to staff and clients. Experience planning and supervising trips and activities. Experience with outdoor skills such as hiking, rock climbing, paddling, skiing, etc. is also preferred.
