Job Summary / Purpose:

The Supervisor of Clinical Services is a full-time leadership position responsible for maintaining the quality and coordination of all clinical services provided by the agency. This role ensures compliance with state and federal home health regulations, participates in policy development, supervises clinical staff, and supports effective program implementation and evaluation. The Supervisor of Clinical Services serves as the lead clinical authority under the direction of the agency administrator.

Essential Duties and Responsibilities:

As required by Connecticut Regulation 19-13-D68(e) 1-3, the Supervisor of Clinical Services shall:

● Have primary authority and responsibility for ensuring the quality of all clinical services delivered by the agency.

● Coordinate and manage all clinical services rendered to patients and families by direct care personnel under their supervision.

● Provide direct supervision of assigned nursing staff in the delivery of skilled services to ensure safe, effective, and compliant care.

● Perform clinical competence evaluations for assigned nursing personnel and participate with other supervisors in the evaluation of all direct care staff, including paraprofessional caregivers.Participate in the development, implementation, and evaluation of agency policies, procedures, standards of care, and objectives that govern clinical services

● Participate in the recruitment, hiring, orientation, in-service education, and training of direct care staff, including nurses and home health aides.

● Collaborate in program planning, budgeting, quality assurance, and performance evaluation activities specific to clinical services.

● Ensure clinical documentation and care planning are timely, accurate, and meet all regulatory standards.

● Maintain effective communication between the Administrator, interdisciplinary team, patients, and families.

● Contribute to interdisciplinary team meetings, care conferences, and case reviews.

● Ensure compliance with all applicable Connecticut Department of Public Health regulations, agency policies, and best practices for home health services.

Required Skills, Education and Certifications:

· The Supervisor of Clinical Services must hold a valid Registered Nurse (RN) license in good standing in the State of Connecticut and meet one of the following qualifications:

o Master’s degree in nursing or public health from a program approved by the National League for Nursing or the American Public Health Association, plus 1 year of full-time clinical experience in a home health agency or community health program involving care of the sick at home. OR

o Bachelor’s degree in nursing and 3 years of full-time clinical experience, including at least 1 year in home health or community health with care of the sick at home. OR

o Continuous employment as Supervisor of Clinical Services in a Connecticut licensed home health agency since before January 1, 1979. OR

o Diploma or associate degree in nursing, plus:

§ 3 years of full-time (or equivalent) clinical experience in nursing within the past 5 years, including 1 year in home health or community health with care of the sick at home, AND

§ Certification by the American Nurses Association in community health or completion of 6 credits in community health nursing theory or healthcare management in the past 2 years.

· Must provide documentation of health clearance and required immunizations.

· Must be able to pass background checks and meet employment eligibility requirements.

· Strong leadership and supervisory skills with the ability to mentor and guide clinical teams.

· Excellent interpersonal, communication, and critical thinking abilities.

· Proficient in clinical assessment, care planning, and documentation review.

· Demonstrated knowledge of Connecticut home health regulations and standards of practice.

· Ability to manage clinical operations, evaluate outcomes, and implement quality improvement initiatives.

· Ability to mentor, evaluate, and develop Home Health Aides

· Skilled in conflict resolution, team-building, and staff performance evaluation.

Physical Requirements:

● Ability to travel locally approximately up to 10% for client visits, staff supervision, and community engagement.

● Ability to lift and carry up to 15-20 pounds

● Ability to sit, stand, walk, and perform repetitive motions for extended periods.

● Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
