Key Responsibilities – RN / Registered Nurse / Home Health
- Perform in-home assessments and develop individualized care plans
- Provide skilled nursing care under physician-directed orders
- Administer wound care, injections, medications, and other treatments
- Accurately document patient status, care plans, and progress notes
- Educate patients and families on procedures and health management
- Collaborate with physicians and care teams to ensure continuity of care
- Coordinate care and lead clinical efforts as an RN Case Manager
- Conduct tasks such as specimen collection, pain management, and disease monitoring
- Ensure accurate ICD-10 coding and documentation for compliance and billing
Qualifications – RN / Registered Nurse / RN Case Manager
- Active RN license in good standing in Indiana
- Minimum of 1 year RN experience (home health or clinical setting preferred)
- Strong interest in growing into a Case Manager or RN Case Manager role
- Skilled in wound care, geriatrics, pediatric care, and behavioral health
- Ability to work independently with diverse patient populations
- Strong clinical judgment, communication, and documentation skills
