What you'll do
- Provide in-home physical therapy services to adult and geriatric patients under physician orders
- Perform comprehensive assessments and functional evaluations; develop and update individualized plans of care
- Coordinate with an interdisciplinary team to align therapy goals with the total plan of treatment
- Initiate PT programs and teach family/caregivers specific techniques for safe carryover at home
- Document timely, accurate visit notes in the EMR; communicate changes in condition promptly
- Prepare clinical/progress summaries; collaborate on safe discharge planning and continuity of care
- Adhere to agency, state, and federal regulations and professional standards of practice
- $100 per hour (experience-based) mileage reimbursement between patient visits
- Flexible scheduling (build a caseload around your availability)

- Part-time / Per Diem (PRN) – choose days/times that work for you
- Service area: Monterey County (Monterey, Carmel, Pacific Grove, Seaside, Marina, Salinas, Pebble Beach, Carmel Valley) and nearby Hollister & Gilroy
- Work location: On the road / patient homes
Required
- Bachelor's or higher in Physical Therapy from an APTA-accredited program
- Current, unrestricted California Physical Therapist PT license
- 2 years professional PT experience
- Current CPR certification
- Valid driver's license and reliable smartphone for EMR use
- Home Health Physical Therapist experience
- Reliable transportation with auto insurance; comfortable traveling within an assigned territory
- Strong assessment, documentation (EMR), and communication skills; able to work independently in the home setting
