Job Summary
We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will work with patients of various ages, including pediatrics and geriatrics, to help them achieve their maximum potential in daily living activities. This role involves developing individualized care plans, conducting assessments, and implementing therapeutic interventions in various settings such as inpatient facilities, skilled nursing facilities, and home health environments.
Duties
- Conduct comprehensive evaluations of patients to assess their functional abilities and needs.
- Develop and implement personalized treatment plans based on patient assessments and goals.
- Collaborate with interdisciplinary teams to coordinate care and ensure optimal patient outcomes.
- Provide education and support to patients and their families regarding treatment plans and home exercises.
- Assist in discharge planning to ensure a smooth transition for patients returning home or moving to other care settings.
- Maintain accurate documentation of patient progress, treatment plans, and case management activities.
- Stay updated on best practices in occupational therapy, particularly in areas such as autism intervention, early intervention strategies, and geriatric care.
Skills
- Strong knowledge of case management principles and practices.
- Proficiency in developing effective care plans tailored to individual patient needs.
- Experience with discharge planning processes for various healthcare settings.
- Familiarity with working with geriatric populations as well as pediatrics, including early intervention techniques.
- Ability to provide therapy in diverse environments such as inpatient units, skilled nursing facilities, and home health settings.
- Excellent communication skills for effective collaboration with patients, families, and healthcare professionals.
- Compassionate approach towards working with individuals with autism and other developmental challenges.
