We are seeking experienced and compassionate Registered Nurses to join our innovative Hospital-at-Home program.Position Overview: As a Home Care RN, you will deliver skilled nursing visits to patients who are formally admitted to the hospital but receiving care at home. You’ll play a vital role in providing high-acuity, patient-centered care while working closely with a coordinated care team and real-time command center support.
Key Responsibilities:- Perform comprehensive head-to-toe assessments - Administer IV infusions and manage infusion therapy - Provide wound care and dressing changes - Conduct lab draws and specimen collection - Monitor patient condition and respond to clinical changes - Communicate patient status updates by phone to a centralized command center during visits - Collaborate with a team of nurses to ensure seamless, around-the-clock patient care Scheduling
Requirements:- Must be available for morning, midday, and evening visits - Daily patient care may be shared among a team of nurses to support continuity and coverage Qualifications:- Active RN license in good standing - Experience in acute care, home health, or infusion therapy preferred - Strong clinical judgment and critical thinking skills - Excellent communication and documentation abilities - Reliable transportation and valid driver’s license
