Job Description:
We are seeking a reliable and compassionate PRN Physical Therapist to provide one-on-one care to patients in their homes throughout Frisco and Carrollton. The ideal candidate is self-motivated, experienced in home health, and eager to help patients reach their full potential.
Key Responsibilities:
- Evaluate, develop, and implement individualized treatment plans.
- Deliver direct physical therapy services to patients in their homes.
- Maintain timely and accurate documentation in compliance with Joint Commission standards.
- Coordinate care with the Director of Nursing.
- Manage a flexible part-time schedule and attend all appointments punctually.
- Supervise and support a growing team of Physical Therapy Assistants (PTAs).
- Degree from an accredited Physical Therapy program.
- Current Texas state licensure as a Physical Therapist.
- At least 1 year of experience as a PT (home health experience preferred).
- Reliable transportation and ability to travel between patient homes.
