Job Overview
- BrightStar Care is seeking a compassionate Certified Nursing Assistant (CNA) to provide one-on-one home health care to clients in their own homes.
- The Certified Nursing Assistant (CNA) will follow an RN-developed care plan and deliver safe, dignified, and high-quality care.
- As a Certified Nursing Assistant (CNA) in home health, you will support clients with daily needs, mobility, safety, and emotional well-being.
- The role requires a dependable and professional Certified Nursing Assistant (CNA) who can work independently while communicating effectively with the clinical team.
- This position is ideal for a Certified Nursing Assistant (CNA) who values flexibility, meaningful work, and building trusted relationships with clients.
Responsibilities
- Provide ADL assistance: bathing, dressing, grooming, toileting, hygiene.
- Assist with safe transfers and mobility using gait belts, Hoyer lifts, slide boards, etc.
- Monitor and report changes in condition to the supervising RN immediately.
- Offer medication reminders as directed in the care plan (no med administration).
- Provide specialized in-home care for clients with:
- Dementia / Alzheimer's
- Developmental disabilities
- Chronic illness
- Memory care needs
- Maintain infection control standards and proper PPE use.
- Document care tasks and observations using BrightStar's mobile EMR/app.
- Follow the RN-created care plan and communicate updates promptly.
- Maintain a safe, clean environment within the client's home.
- Provide companionship, emotional support, and promote client independence.
- Protect client confidentiality and uphold HIPAA regulations.
Experience & Requirements
- Active California Certified Nursing Assistant (CNA) certification required.
- BLS/CPR preferred.
- Prior home health CNA or in-home care experience strongly preferred.
- Experience with seniors, dementia clients, or individuals with disabilities is a plus.
- Ability to work independently in client homes and manage time effectively.
- Comfort using mobile apps for charting (training provided).
- Reliable transportation and valid driver's license (if required).
