The RN will be responsible for conducting in-home patient assessments, performing skilled nursing visits, and developing individualized care plans in collaboration with the interdisciplinary care team. This position plays a critical role in ensuring high-quality, patient-centered care delivered in the comfort of our clients' homes.
Key Responsibilities:
- Perform comprehensive patient assessments in the home environment, including physical, psychosocial, and environmental assessments.
- Develop and implement individualized care plans based on patient needs, physician orders, and best practices.
- Provide direct nursing care, including wound care, medication administration, catheter care, injections, and other skilled nursing tasks.
- Monitor patient condition and progress; communicate any changes to the physician and care team.
- Educate patients and caregivers on disease process, medications, self-care techniques, and health maintenance.
- Accurately document all assessments, interventions, and communications in the electronic medical record (EMR) system in a timely manner.
- Collaborate with multidisciplinary team members, including physicians, physical therapists, occupational therapists, and social workers.
- Ensure compliance with all state and federal regulations and agency policies.
- Participate in case conferences, staff meetings, and ongoing training as required.
- Current Registered Nurse (RN) license in MA
- Minimum 6 months to 1 year of nursing experience, preferably in home health, community health, or acute care settings with assessments.
- CPR certification (BLS) required
- Strong clinical and assessment skills
- Excellent communication, time management, and organizational skills
- Valid driver's license and reliable transportation
- Ability to work independently and travel to patient homes
- Experience with EMR systems preferred
